The TUE forms can be downloaded here:
- TUE Application Form (Word Format)
- Checklist to assist applicants ensure that all the required documentation is included with their application. Please NOTE that all documents must be in English.
If you have difficulty in downloading either of these files, please email communications@worldbridgefed.com and request a copy to be sent by email.
Note these very important points:
- Only players who use medication listed on the prohibited list need to apply for a TUE. Please check first before applying. The current Prohibited List can be found here.
- Any players requiring exemption certificates should ensure that the Therapeutic Use Exemptions (TUE) Application Form is fully completed in English.
- Your physician MUST complete the diagnosis section (sections 4 & 5) fully and accurately with the following documentation which must all be in English:
- Evidence confirming the diagnosis must be attached and forwarded with this application. The medical information must include a comprehensive medical history and the results of all relevant examinations, laboratory investigations and imaging studies. Copies of the original reports or letters should be included when possible. In addition, a short summary that includes the diagnosis, key elements of the clinical exams, medical tests and the treatment plan would be helpful.
- If a permitted medication can be used to treat the medical condition, please provide justification for the therapeutic use exemption for the prohibited medication.
If this information is not provided, or is provided in a language other than English at the time of application your TUE will not be issued.
Once the form is completed you should send it, together with all the documents provided by your physician, to the Chairman of the TUE Committee, Dr Jaap Stomphorst (click here to email) as soon as possible and no later than 30 days before the commencement of the competition in which you are participating. It is acceptable to scan and email the TUE form and documentation.
Instructions how to apply for a TUE can be find on the WADA website
In the same location you can find TUE-checklists about specific diseases

TUEs from other National Anti-Doping organisations or recognised Sports Organisations
Please be aware that the WBF does not automatically recognise a TUE awarded by any other organisation (i.e. a NADO or ANY other recognised sports organisation). Thus in the absence of such automatic recognition, the Athlete shall submit a request for recognition of the TUE to the WBF to the Chairman of the TUE Committee, Dr Jaap Stomphorst (click here to email) . The request should be accompanied by a copy of the TUE and the original TUE application form and supporting materials referenced at Articles 6.1 and 6.2 (unless the Anti-Doping Organization that granted the TUE has already made the TUE and supporting materials available via ADAMS or other system approved by WADA, in accordance with Article 5.4)
Players are advised that the WBF may select samples following testing for long term storage for analysis in future years.
