Note that all International Level athletes must apply for a TUE if they are taking any prohibited medication and ensure that it is kept up to date at all times.
It is important that your doctor not only fills in the Therapeutic Use Exemptions (TUE) Application Form but supplies additional information as required and this should be noted carefully and included with the form.
Full details can be found on the WADA (World Anti-Doping Authority) website, but note that section 8.9 states:
The application shall include a comprehensive medical history and the results of all examinations, laboratory investigations and imaging studies relevant to the application. The arguments related to the diagnosis and treatment, as well as duration of validity, should be guided by the WADA “Medical Information to Support the Decisions of the Therapeutic Use Exemption Committees (TUECs)."

It would be best if you print any document on the Medical Information pages relating to your personal medical condition, and take them to your Doctor when applying for a TUE so that he or she can provide the correct and full information needed at the time you apply.
